Indonesian investigators said on Wednesday a Lion Air Boeing 737 jet that plunged into the sea, killing 189 people on board, was not airworthy on a flight the day before it crashed on Oct. 29, Reuters reports. A malfunctioning sensor and an automated response from the aircraft’s software stymied pilots’ efforts to control a doomed Indonesian flight that went careening into the sea last month, according to a preliminary investigative report quoted by The Washington Post. Interviews with dozens of Lion Air’s management personnel and flight and ground crew members, as well as Indonesian investigators and airline analysts, paint a picture of a carrier so obsessed with growth that it failed to build a proper safety culture, according to The New York Times.
